How to Schedule a First Visit:
- Call 518-621-5110 or email officemanager@elevatenaturopathic.com with a request to schedule a new patient/client visit. Please provide your name, phone number, date of birth and email address. You will be sent a link to the patient portal. In order to schedule a visit, you must fill out all of the intake paperwork PRIOR to being able to schedule. Dr. Cole will go through the paperwork to be sure you are a good fit for her practice. After Dr. Cole has had a chance to review your paperwork, you will be contacted to schedule a visit.

- Bring in or have sent to the office any copies of bloodwork, imaging and other testing done within the past year. You can also upload this through the portal. Here is a tutorial on how to use the portal system.
- Bring in any supplements/medications that you may already take with you.
- Please double check that you are scheduling your appointment at the location you desire.
- Please read the directions to the office so you do not get lost.
